Service DescriptionPeople who are eligible attend day program once weekly from 9-2 pm.
* helps adults in the downtown and Point Douglas area to remain living in the community.
* provides rehabilitation and education to support recovery from illness or injury and prevent complications from chronic disease.
Participants attending group exercise program, are provided with a hot lunch and participate in a group activity in the afternoon. A daily education session is also provided that includes information on medication management, falls prevention, maintaining general health and community information. Based on need and assessment, recommendations may also be provided by physio, OT, dietitian, nurse or social work. The team will also work with the primary care provider such as family doctor or nurse practitioner.
Accessible transportation is available to participants who are not able to make arrangements themselves.
