Service Description

* is designed to address the needs of older adolescents who are transitioning into adulthood and increased independence

* consists of services for youth learning to live on their own or in some circumstances, with their own children

* requires the youth to consistently participate in an educational program, employment, or a combination in order to maintain their placement with the LIFE program

* operates 2 different program streams including a transition house and individual apartments

* provides all youth involved in this program with a minimum of 15 support hours per week

* provides some clients with higher levels of support to meet the expectations of the program, and work toward their care plan goals

* determines support hours based on individual needs and plans

* the support hours are typically used to assist clients with budgeting, shopping, child care, scheduling/attending appointments and school/employment commitments
